TEENLIFTING d.o.o. from Zagreb, Nečujamska br.3., OIB: 80188343863, TEENLIFTING CENTER from Zagreb, Nečujamska br.3 OIB: 14350602570 and TEENLIFTING franchises – third parties who have signed contract – TEENLIFTING franchise agreement (hereinafter: TEENLIFTING®), in collecting and processing personal data acts in accordance with the principles of legality, fairness and transparency, purpose limitations, data reduction, accuracy, storage limitations and the integrity and confidentiality of personal data of respondents, is handled carefully and in accordance with Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 on the protection of individuals concerning the processing of personal data and the free movement of such data and the repeal of Directive 95/46 / EC (General Data Protection Regulation, hereinafter: the Regulation) and the applicable regulations of the Republic of Croatia on personal data protection, with regard to data collected and processes, in order to comply with our obligations regarding the protection of personal data as a controller.
I am familiar with the fact that by filling out the forms “Contact Us” / alt “INQUIRY” and / or “Become part of the TEENLIFTING story!” / alt “COOPERATION REQUEST”on the TEENLIFTING® website, I expressly and voluntarily give my CONSENT, by which I confirm that I agree that TEENLIFTING® collects, processes and uses all my personal data listed in the form “Contact Us” / alt “INQUIRY” and / or “Become part of the TEENLIFTING story!” / alt “COOPERATION REQUEST”.
I confirm that I am aware that TEENIFTING® will use and process the above personal data for the purpose of providing information on the provision of TEENLIFTING® electrostimulation services for facial and body muscles as well as other cosmetic services in its field.
By signing the Consent “I want to receive TEENLIFTING® notifications” I declare that I agree that my personal data listed in the form “Contact Us” / alt “INQUIRY” and / or “Become part of the TEENLIFTING story!” / alt “COOPERATION REQUEST” stored in the database for contact purposes for possible notification of periods of favorable prices, the opening of TEENLIFTING® franchises centers in new cities, as well as for the purpose of marketing activities. TEENLIFTING® processes and stores the collected data only during the period in which the above consents have not been withdrawn.
Personal data from this Consent will not be passed on to third parties and will be treated confidentially and in accordance with the Regulation and applicable regulations of the Republic of Croatia on personal data protection, and TEENLIFTING® takes appropriate measures to ensure personal data security and compliance with applicable data protection regulations. , such as the Regulation and the Law on the Implementation of the General Regulation on Data Protection. TEENLIFTING® conscientiously protects the personal information contained in these Concessions from loss, destruction, misuse, unauthorized access and unauthorized disclosure. All persons employed in TEENLIFTING® are obliged to respect the confidentiality of personal data from these Concessions and to comply with the general acts and decisions of TEENLIFTING® that regulate data protection issues. I also explicitly confirm that, when providing the above personal data, I have been informed of the following and that I have understood the information provided:

(b) my rights regarding the protection of my personal data for which I grant these Concessions are:
- I have the right to access my personal data collected under these Concessions, which means the right to receive from TEENLIFTING® information related to the process of collecting and processing personal data from this Consent and to receive confirmation of my personal data and a copy of my personal data if it does not adversely affect the rights of other respondents
- I have the right to correct and supplement inaccurate personal data collected under these Concessions.
- I have the right to “forget”, ie to delete personal data collected on the basis of these Concessions
- I have the right to restrict the processing, ie that personal data is not used but kept if the accuracy of personal data is disputed, if the processing is illegal or during the resolution of the complaint procedure
- I have the right to the portability of data given on the basis of this consent, which means the right to receive personal data from TEENLIFTING® in order to pass it on to another controller
- I have the right to object to data processing
- I have the right to file a complaint to the Personal Data Protection Agency in the event that TEENLIFTING® acts contrary to the Regulation when processing personal data collected on the basis of this consent.
Finally, I am aware that I can send all my requests regarding these rights in writing to the address TEENLIFTING d.o.o or TEENLIFTING CENTER, Nečujamska br.3, 10000 Zagreb or via e-mail to privacy@teenlifting.eu.
I have been informed that TEENLIFTING® may request personal information from me for the purpose of reliable identification in the event of exercise of rights, and that it may reject the request only if it proves that it cannot reliably establish my identity.
I am aware that TEENLIFTING® must respond to my request for the exercise of rights, related to the above personal data, without undue delay and in any case within one month of receiving the request.
